This presentation examines a challenging question in hip preservation: when a patient has hip dysplasia, hip pain, and some degree of arthritis, is periacetabular osteotomy (PAO) still an option, or is total hip replacement more appropriate? The discussion reviews how age, joint-space preservation, Tonnis grade, cartilage damage, and previous hip surgery may influence outcomes, while highlighting the uncertainty surrounding which arthritic hips can improve after PAO.
